Juvenile osteochondritis dissecans (JOCD) affects children during a critical period of physical, social and emotional development. Months to years of activity restriction, while clinicians wait to see whether lesions heal, can prevent participation in sports and lead to social isolation, decreased self-esteem and significant family stress. Ethically, prolonged “watchful waiting” may delay needed decision‑making and extend harm.

New MRI techniques, however, offer a promising path forward. Quantitative T2* mapping can reveal early biological signs of healing or non‑healing that are not detectable on standard imaging. By identifying children unlikely to improve with non‑operative care, clinicians can make earlier, more informed decisions that support truly patient‑centered care.
